3. SMS/Text Messaging Terms & Consent

By providing your mobile phone number during the appointment booking process, you explicitly consent to receive transactional SMS/text messages from WiseCare related to your appointment.

  • Purpose: Messages will be used strictly for appointment confirmations, pre-visit instructions (such as requests to complete mandatory forms like MedXpress), and appointment reminders. We do not use your phone number for promotional marketing without separate, explicit consent.
  • Message Frequency: Message frequency varies depending on your appointment status and required communications.
  • Rates: Standard message and data rates may apply depending on your mobile carrier plan.
  • Opt-Out: You may opt-out of receiving SMS messages at any time by replying STOP to any message you receive from us. After texting STOP, you will receive one final message confirming your opt-out. Note that opting out may impact our ability to send you timely appointment updates.

4. Third-Party Services

For USCIS Immigration Physicals, we do evaluate individuals under the age of 18; however, all bookings and medical form preparations (Part 4 of Form I-693) must be completed by a legal guardian or authorized preparer.
