About Your Examination

Board-certified physician Perry Weisman, MD, is a designated Civil Surgeon authorized to perform comprehensive immigration medical exams in accordance with United States Citizenship and Immigration Services (USCIS) requirements at his Pasadena, Maryland office at WiseCare Urgent Care. He has performed USCIS exams for over 10 years for thousands of applicants.

Form I-693 Instructions

  • Preparation: The I-693 Part 7 will be pre-filled out. You are required to fill out Part 1 and Part 2. Do not get further labwork or vaccinations before your visit; the Civil Surgeon will instruct you on exactly what is required.
  • Interpreter/Preparer: You have the option to fill out Part 3 if using an interpreter and Part 4 if using a preparer (Part 4 is required if under 18).
  • Signatures: You are required to sign NOT DATE Part 2. Interpreter must sign NOT DATE Part 3. Preparer must sign NOT DATE Part 4.
  • Submission: Print the entire form. All signatures must be in original ink. The pre-filled form should be sent securely to wisecaremd@gmail.com.
