1. Acceptance of Terms

By accessing our website and booking an appointment with WiseCare Administrative and Executive Exams ("WiseCare," "we," "us," or "our"), you agree to be bound by these Terms and Conditions. If you do not agree with any part of these terms, you must not use our website or services.

2. Services Description

WiseCare provides specialized medical examinations and primary care services, including:

• USCIS Immigration Physicals: Comprehensive medical exams for Form I-693 by a designated Civil Surgeon.
• FAA Medical Exams: 1st, 2nd, and 3rd class physicals performed by an authorized Aviation Medical Examiner.
• DOT Physicals: Commercial driver physicals adhering to FMCSA guidelines.
• Direct Primary Care (DPC): Primary care services available on a fee-for-service basis or via monthly membership.

3. Appointment Booking and Deposits

To reserve an appointment slot, patients are required to pay a non-refundable deposit upfront via our secure online booking system. The deposit is applied toward the total cost of your visit.

• USCIS: $100 deposit towards the $300 total cost.
• FAA: $100 deposit towards the $225 total cost.
• DOT: $100 deposit towards the $175 total cost.
• Direct Primary Care: $100 deposit towards the $195 total cost of the visit. For established DPC members, a $75 upfront payment is required to hold the appointment slot.

The remaining balance is due at the time of your visit.

4. SMS/Text Messaging Consent

By providing your mobile phone number and booking an appointment, you explicitly consent to receive transactional text messages from WiseCare.

• These messages are required for the administration of your appointment, including booking confirmations, links to mandatory pre-visit forms, and schedule reminders.
• Message frequency varies based on your appointment activity.
• Message and data rates may apply through your mobile carrier.
• You may opt out at any time by texting STOP. For assistance, text HELP or contact our office.
• Our SMS services are compatible with major US carriers. Carriers are not liable for delayed or undelivered messages.

5. Payment Terms and Cancellation Policy

Deposits collected at the time of booking are strictly non-refundable to ensure schedule integrity and availability for other patients requiring regulatory exams. If you need to reschedule your appointment, please contact our office at least 24 hours in advance. Failure to show up for an appointment without prior notice will result in the forfeiture of your deposit.

6. Medical Disclaimer

The content provided on our website is for informational purposes only and does not constitute medical advice. Booking an appointment does not establish a physician-patient relationship until you are evaluated in person by Dr. Perry Weisman. For regulatory exams (USCIS, FAA, DOT), the physician's role is strictly evaluative to determine compliance with federal guidelines; these exams do not replace routine medical care provided by your primary care physician (unless you are enrolled in our Direct Primary Care program).

7. Limitation of Liability

To the maximum extent permitted by law, WiseCare shall not be liable for any indirect, incidental, special, consequential, or punitive damages resulting from your use of our website or your inability to secure regulatory clearance (e.g., medical certificate denial by the FAA or FMCSA), as these decisions are governed by federal medical standards and agency review.

8. HIPAA Compliance

9. Governing Law

These Terms and Conditions shall be governed by and construed in accordance with the laws of the State of Maryland, without regard to its conflict of law principles.
